What You'll Do
As a Family Support Coordinator, you will be the ultimate bridge to vital resources, community connection, and advocacy:
- Advocate & Empower: Connect families to state and community resources while maximizing their control over their own services.
- Guide Transitions: Help individuals smoothly plan and navigate the massive milestone of transitioning from school to adult services.
- Navigate Legal & Financial Pathways: Assist with budget administration and facilitate important processes like Guardianship and Power of Attorney.
- Collaborate & Document: Facilitate team planning meetings and maintain meticulous documentation alongside a passionate, friendly team.
We need a flexible, creative, and non-judgmental professional who genuinely loves serving diverse families.- Education: A Bachelor's degree is highly preferred, but extensive case management experience will absolutely be considered instead.
- Experience: At least 2 years in human services, with a minimum of 1 year in family support. Experience with developmental disabilities is a huge plus!
- Skills: Excellent communication, strong organization, intermediate MS Office proficiency, and a willingness to learn new programs.
- Travel: Reliable transportation for regional/statewide home visits and meetings.
- Competitive Pay: $22.00 to $25.00 per hour, depending on experience.
