Responsibilities As part of the person-centered care model, this role will: Develop and coordinate agency-wide programs for prevention and management of skin breakdown. Provide expert consultation or hands‑on care for patients with acute or chronic wounds. Establish and coordinate multi-disciplinary comprehensive management plans. Coordinate care for patient populations, identifying trends and developing standardized prevention and treatment protocols, refining them based on outcomes. Develop and implement educational programs for patients, staff, and other healthcare providers. Coordinate quality improvement initiatives to optimize skin and wound outcomes. Advocate for patients with skin and wound care needs. Manage regional wound care program metrics to ensure consistent delivery of quality care and explore cost‑effective wound treatment opportunities. Review quality reporting data for deficiencies, areas for improvement, and barriers to safety. Seek consultation with regional WOCN staff to identify and plan interventions for complex client care problems. Develop an interdisciplinary approach to client care between the Wound Program, HHCAH clinical staff, and community partners. Inform physicians and wound clinics of changes in client needs or wounds requiring adjustments to the plan of care. Follow trends in specialty wound care for the home‑care industry and recommend treatment options for program incorporation. Participate in reviewing regional wound program performance metrics with the Regional Director and Manager of Specialty Services. Participate in training, coaching of staff, and onboarding to the Wound program. Facilitate professional development needs and competencies for the wound program. Additional Duties Actively participate in the performance improvement process known as H3W. Provide clinical education programs to improve direct service staff competencies. Research new information related to wound specialty and communicate to the team. Perform other duties as assigned. Qualifications Education: Bachelor’s Degree in nursing required; WOCN certification required; MSN preferred. Experience: Minimum two years nursing experience in a clinical specialty area required; two years in homecare preferred. Licensure, Certification, Registration: RN required; obtain Wound/Ostomy certification within six months of hire. Language Skills: Strong written and verbal communication skills. Ability to understand problem situations, solve problems, and independently assess a wide variety of tasks to identify solutions benefiting the client/agency. Knowledge of relevant industry standards for wound care treatments. Ability to balance financial, quality, people, and customer expectations appropriate to business situations. Intermediate to advanced proficiency in Microsoft Outlook, Word, Excel, PowerPoint, and Project.
