Mobile Clinical Care Manager - Dual-Eligible Care Lead

The Clinical Care Manager position is a mobile role working across Massachusetts to monitor and coordinate care for dually eligible enrollees with complex needs. You will engage with members in homes and community settings, lead the care team, and coordinate with providers to ensure seamless transitions and appropriate services.
