TITLE
Registered Nurse - Home Health

SCHEDULE
Shift: 8 hours

ASSIGNMENT DETAILS
Contract Length: 14 weeks
Guaranteed Hours: 40

REQUIREMENTS
- Current state license as a Registered Nurse
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years of home health experience
- Homecare Homebase Software Experience
- OASIS Experience
- Proof of current CPR certification
- Current driver's license
- Must have reliable transportation and insurance

ROLES & RESPONSIBILITIES
- Assess home health and/or hospice patients to identify physical, psychosocial, and environmental needs.
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Educate patients and their families on care, diet, medications, and home safety while promoting self-care practices.
- Supervise and provide clinical direction to support staff to ensure quality and continuity of service.
- Monitor assigned cases for compliance with third-party payer requirements.
- Serve as a liaison to patients/families, team members, administration, and other healthcare professionals.
