Job Description

As an RN Case Manager in hospice care, you will experience satisfying and challenging work that makes a difference every day. You will become an integral part of your patients' care teams, providing broad independence while working collaboratively with a cohesive interdisciplinary team. This position offers a work/life balance with the flexibility you need.

Responsibilities

• Develop and implement a care plan in collaboration with the patient's physician, the patient, and their family or primary caregivers upon admission.
• Periodically review and recertify the patient care plan as appropriate.
• Coordinate the patient's care plan through interdisciplinary communication and case conferences.
• Conduct ongoing assessments of the patient's clinical, functional, and emotional status to identify problems or progress toward desired goals.
• Communicate significant changes to the patient's physician and colleagues.
• Document evaluations, patient responses, and progress toward goals.
• Teach patients and caregivers about care techniques and end-of-life symptom management.

Required Experience / Certifications / Licensure

• Must be a licensed Registered Nurse in Vermont and New Hampshire or hold an unencumbered compact license.
• Graduate of an accredited school of nursing.
• Medical/Surgical/Oncology nursing and/or hospice care experience preferred.
