Job Overview Functional Title: Case Manager Job Title: Social Worker II Agency: Dept of State Health Services Department: Region 11 Location: MERCEDES, TX Posting Number: 17537 Salary Range: $4,020.33 - $5,178.00 per month Job Summary Under the supervision of the Program Supervisor III, this position provides advanced and comprehensive case management consultation for families with children who have health risks, conditions, or special health care needs. The incumbent oversees case management services for families in the Children with Special Health Care Needs (CSHCN), Community First Choice (CFC), Newborn Screening (NBS), Personal Care Services (PCS), and Case Management for Children and Pregnant Women programs. Responsibilities include travel for home visits, completion of intakes, family needs assessments, service plans, and follow‑up services; conducting presentations; providing referral services; and completing administrative duties such as reporting and coordination with community organizations. The role supports public health staff during regional or state disaster events and requires up‑to‑50% travel and use of personal transportation. Essential Job Functions Case Management Functions (45%) : Provide complex and specialized social work consultation and comprehensive case management services including intake, family needs assessment, service plan, and follow‑up for families in the CSHCN, SSI, NBS, PCS, CFC, and Case Management for Children and Pregnant Women programs. Conduct home visits, maintain documentation, and use relevant software tools. Technical Assistance/Quality Assurance/Training (20%) : Deliver consultation, presentations, and training to agencies and staff; assist in training new staff, volunteers, and interns; conduct peer chart reviews for quality assurance. Community Organization/Resource Development (20%) : Participate in community functions, collaborate with other entities, represent DSHS on the CRCG and related groups, and maintain a social service directory. Professional & Team Development (15%) : Attend trainings and conferences, provide backup to team members, support disaster relief efforts, and participate in performance evaluations and staff meetings. Knowledge, Skills, and Abilities Case management principles and practices. Children’s programs such as Medicaid, CSHCN, CPW, SSI, NBS, Lead, Early Childhood Intervention. Quality assurance and technical assistance policies and procedures. Community resources at local, state, and national levels. Cultural and socioeconomic issues related to health and disabilities. Computer skills: MS Word, PowerPoint, Excel, Access, Outlook, and database entry. Preparation of monthly reports, travel reports, and time/activity reports. Conducting in‑home family needs assessments and developing service plans. Providing presentations and training. Conflict resolution, mediation, and problem solving. Team collaboration, application of program policies, professional leadership, confidentiality, case‑management planning, and independent judgment. Adherence to DSHS policies, conduct standards, and work rules. Licensure and Requirements • Current permanent license to practice as a social worker in Texas (LBSW, LMSW, LMSW‑AP, or LCSW). • Valid Class C Texas Driver’s License or equivalent. • Bachelor’s or master’s degree in Social Work from an accredited institution. • Good standing with the Texas Social Work Licensing Board. • Bilingual proficiency in English and Spanish. • Willingness to travel a minimum of 50% and have own transportation.
