Our vision is revolutionizing how healthcare professionals produce, access, and utilize information and thus enabling them to focus on the core value of their work: patient outcomes.This is a non-bedside role for a Licensed Practical Nurse (LPN) to support patients enrolled in Medicare’s Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs.**Your Contribution:*** Create, update, and review patient care plans based on the provider’s plan of care* Conduct monthly telephonic check-ins with patients to assess health status and medication adherence* Review RPM synced vital signs and escalate concerns to the provider when appropriate* Coordinate follow-ups, scheduling, and documentation updates* Ensure all documentation meets Medicare CCM and RPM compliance guidelines* Accurately document all patient interactions within multiple EHR systems**Your Qualification:*** Active LPN License (Multistate Nurse Compact License Required)* Strong verbal and written communication skills* Minimum 1 year in related Chronic Care Management services* Experience working with EHR systems* Ability to work independently with minimal supervision* Reliable high-speed internet and a quiet, HIPAA-compliant workspace**What you can expect from us:*** **Purpose**: Become a part of a significant initiative.
