Your expertise will enable you to assess, treat, and educate patients with a wide range of diagnoses and conditions. You will also collaborate with our healthcare team to ensure seamless communication and coordination of care. Responsibilities:
- Assess and treat patients with a variety of physical disabilities, injuries, and conditions
- Develop and implement individualized rehabilitation plans that promote optimal recovery and independence
- Conduct evaluations, progress reports, and discharge summaries
- Provide education and training to patients, families, and caregivers on proper techniques and strategies for maintaining strength, flexibility, and mobility
- Collaborate with our healthcare team to ensure comprehensive care and coordination
- Maintain accurate and timely records of patient assessments, treatments, and progress
- Degree in Physical Therapy
- Current licensure as a Physical Therapist in the state of Michigan
- Minimum of 0 years of experience in acute care, post-acute care, or long-term care settings
- Strong communication and interpersonal skills
- Ability to work effectively as part of an interdisciplinary team
