Asante is seeking a compassionate and dedicated Nurse Practitioner to join our Outpatient Palliative Care Team . Based in the heart of Southern Oregon , this position offers the unique opportunity to provide holistic, patient-centered care to individuals with serious or life-limiting illnesses in the comfort of their homes .

What You ll Do:

• Deliver comprehensive palliative care in an outpatient and home-based setting.
• Perform thorough patient assessments , develop and implement individualized care plans , and provide symptom management.
• Prescribe medications , order and interpret diagnostic tests, and provide counseling and education to patients and families.
• Collaborate closely with a multidisciplinary team , including physicians, nurses, social workers, chaplains, and other healthcare professionals.
• Support patients physical, emotional, spiritual, and social needs during some of the most critical times in their lives.
• Participate in quality improvement, education, and research initiatives to enhance palliative care services.
