• Schedule: Monday–Friday, 8am–5pm (considering 3–4 days/week)
• Duration: Ongoing, minimum 2 weeks per month

• Experience: Geriatric care experience preferred, minimum 2 years in outpatient setting

What You’ll Do

• Provide outpatient primary care services to adults aged 55+ with a focus on geriatric medicine
• Conduct community and in-home visits, including driving to patient residences in assisted living, SNFs, or private homes
• Collaborate with an 11-member interdisciplinary team to coordinate patient care
• Manage patient conditions through EMR documentation and team communication
• Participate in orientation and ensure compliance with licensing and certification requirements
• Support patients with chronic disease management and preventive care strategies

What You Need

• Active medical license in Oregon
• DEA registration
• BLS certification
• Geriatric or outpatient primary care experience (minimum 2 years preferred)
• Strong communication skills and patient-centered approach

About the Assignment
This role involves working at a facility providing multidisciplinary outpatient care to elderly adults, primarily through home visits and community engagement. The provider will work closely with a dedicated team to deliver high-quality, coordinated care in a supportive environment, focusing on improving seniors’ health and quality of life. Most patient management occurs via electronic medical records, with an emphasis on compassionate, comprehensive geriatric care.
