Conducting comprehensive assessments upon patient admission to develop individualized care and discharge plans. Coordinating safe and timely discharge processes, including securing authorizations, arranging transportation, and facilitating transfers. Monitoring patient progress through daily reviews and participating in multidisciplinary rounds to address barriers to discharge. Communicating effectively with physicians, patients, families, and staff to coordinate transitions of care. Qualifications & Skills Master’s Degree in Social Work. Less than 1 year of experience in an MSW internship program. Ability to interact with diverse populations and set priorities efficiently. Knowledge of computer applications and analytical tools. Proven leadership skills. Preferred: Familiarity with EHR systems like EPIC; certifications such as ACM or CCM within 3 years. Additional Information Physical ability to perform activities including lifting, kneeling, bending, and CPR if required.
