Licensed Practical Nurse (LPN)

Position Overview The Licensed Practical Nurse (LPN) is a licensed medical professional cross‑trained to perform routine administrative and clinical tasks to keep the medical centers running smoothly. The LPN supports physician‑lead care teams that provide primary care for a specific patient population. ESSENTIAL JOB DUTIES / RESPONSIBILITIES Measure and record patient vital signs; record patient interview and medical history. Catalog and communicate patient needs to the PCP, clinician, specialist and other clinical personnel. Prepare patients for examinations and perform routine screening tests; assist physicians with exams; explain treatment procedures and physicians’ instructions to patients. Observe patients and report any changes in patient conditions to the PCP, clinician, specialist or other clinical personnel. Perform non‑critical medical and therapeutic procedures based on medical instructions. Collect phlebotomy and other lab specimens; perform basic waived lab tests. Prepare and administer medications; change dressings, apply bandages, remove sutures and perform other first‑aid procedures; use CPR skills when necessary, all under physician supervision. Maintain supplies, equipment, stocks and sterilize instruments; practice OSHA safety standards; perform accurate, legal and ethical documentation at all times. Process patient phone messages, return calls and route them to other team members as appropriate; call patients to obtain and relay pertinent information for the physician. With physician approval, call in prescriptions to the pharmacy and use the ChenMed Rx system to support medication dispensing. Provide health coaching to a defined group of patients to support healthy lifestyle choices; follow up with coached patients via weekly calls. Perform other duties as assigned and modified at the manager’s discretion. KNOWLEDGE, SKILLS AND ABILITIES Knowledge of medical products, terminology, services, standards, policies and procedures. Excellent interpersonal and customer service skills with compassion and empathy toward patients and families. Exceptional oral and written communication skills, time‑management and organizational skills. Ability to collaborate with physicians, clinicians, managers, nurses, case managers, front‑desk staff and other team members. Mindset focused on solving problems for patients and achieving team goals. Skilled in basic phone and computer operation. Detail‑oriented to ensure accuracy of reports and data. Ability to maintain effective and organized systems to ensure timely patient flow. Ability to act calmly in busy or stressful situations. Proficient in Microsoft Office Suite (Word, PowerPoint, Outlook, Excel) and other word‑processing, spreadsheet, database, e‑mail and presentation software. Willingness to travel locally or regionally up to 80% of the time to assist in covering other centers as needed. Spoken and written fluency in English. EDUCATION AND EXPERIENCE High school diploma or equivalent (required). Graduation from a nationally accredited school for practical or vocational nursing (required). Current, active LPN license to practice in state of employment (required). IV Therapy Certification, if required by state Board of Nursing (required). Minimum of 1 year of LPN experience (required). Minimum of 3 years overall clinical experience (preferred). Experience working with geriatric patients (plus). EMR system experience (preferred). Basic Life Support (BLS) certification from the American Heart Association or American Red Cross (required within first 90 days). COMPENSATION Hourly pay range $20.20 – $28.83. Final compensation depends on experience, education, geographic location and other factors. The position may also be eligible for bonuses or commissions.
