LPN - Clinical Care Coordinator II - FPG Family Practice, Venice

The Clinical Care Coordinator II demonstrates competency to provide patient care according to age‑specific population needs and assists in managing patient care in the physician office setting under the direct supervision of a provider. Qualifications Basic Life Support / CPR certification required (must be obtained within thirty (30) days of hire/transfer date). Graduate of an accredited nursing school or a diploma program. Preferred: one (1) year of clinical experience in outpatient physician office practice. Preferred: experience with an electronic practice management system and electronic health record. Preferred: demonstrated knowledge of electronic practice management and electronic health records software along with skill in all Microsoft Office programs (Word, Excel, Outlook, PowerPoint). FL LPN (Licensed Practical Nurse) required.
