Physical Therapist, Skilled Nursing Facility (Travel) — Alameda, California
Start date 08/03/2026 | Contract duration 14 weeks | Weekly pay $2,461–$2,609 per week | Guaranteed hours 35 per week | Shift 08:00–16:30
This travel temporary assignment involves on-site physical therapy services in a skilled nursing facility. You will work as part of an interdisciplinary team to evaluate residents, develop treatment plans, deliver skilled interventions, and help residents regain or improve mobility and independence.

Responsibilities
- Assess residents’ physical therapy needs; establish individualized treatment plans with measurable goals.
- Deliver evidence-based PT interventions, including gait training, therapeutic exercises, manual therapy, balance and core stability work, and functional activity training.
- Monitor progress, adjust treatment plans as needed, and coordinate discharge planning with the interdisciplinary team.
- Document evaluations, progress notes, treatment plans, and outcomes in the patient record in a timely and compliant manner.
- Educate residents and caregivers on home exercise programs, safety, and fall-prevention strategies; ensure smooth transitions of care.
- Maintain patient safety, adhere to infection control practices, and comply with regulatory standards and facility policies.

Requirements
- Doctor of Physical Therapy (DPT) degree or equivalent
- Active Physical Therapist license in California or eligibility for CA licensure
- CPR/BLS certification
- Ability to work on-site in a skilled nursing facility setting and document care per policy
- Willingness to travel for a 14-week assignment starting 08/03/2026
- Strong communication, teamwork, and documentation skills
- Preferred: experience with geriatric populations or SNF settings
