About Family Medicine at Greenhill

Family Medicine at Greenhill (FMAG) is an independent community primary-care practice that has served Wilmington for more than 26 years. We have two established locations and care for patients across the lifespan, from infants and children through adults and older adults.

We are expanding provider capacity so our patients can obtain timely same-day, walk-in, preventive, and chronic-disease care without unnecessary delays. We are looking for two Family Nurse Practitioners, ideally with one clinician primarily aligned to each location.

The Opportunity

This role is designed for an FNP who enjoys comprehensive outpatient care, continuity, chronic-disease management, and same-day access. Experienced clinicians are preferred, but we are willing to invest in the right new graduate who demonstrates strong clinical judgment, communication, work ethic, and willingness to learn.

Candidates should be comfortable caring for a broad primary-care population. Pediatric and geriatric experience are particularly valuable because FMAG’s patient population spans the lifespan.

Core Responsibilities

• Provide preventive, acute, chronic-disease, and follow-up primary care across the lifespan within training and scope.

• Evaluate same-day and walk-in concerns while maintaining continuity for established patients.

• Manage common outpatient conditions such as hypertension, diabetes, respiratory complaints, infections, medication needs, musculoskeletal complaints, preventive-care gaps, and chronic-disease follow-up.

• Perform focused history and physical examinations, develop evidence-based treatment plans, prescribe appropriately, and provide patient education.

• Order and interpret appropriate laboratory testing, diagnostic studies, imaging, and referrals within scope.

• Perform office-based procedures and point-of-care testing consistent with training, competency, and FMAG protocols.

• Participate in preventive-care, annual wellness, chronic-care, quality, and population-health initiatives as appropriate.

• Collaborate with physicians, PAs, behavioral health, nursing, medical assistants, CCM staff, scribes, and administrative team members.

• Provide telehealth and occasional extended-hour or Saturday coverage as mutually scheduled.

• Document visits accurately and timely and participate in continuous improvement of clinical workflows.

Required Qualifications

• Graduate of an accredited Nurse Practitioner program; Family Nurse Practitioner preparation is strongly preferred for an all-ages role.

• Current or eligible Delaware APRN/NP licensure and prescriptive authority.

• National board certification appropriate to the role.

• DEA registration and Delaware controlled-substance authority when required for assigned duties.

• Current BLS certification or ability to obtain before start date.

• Ability to obtain and maintain payer credentialing.

• Strong written, interpersonal, and clinical communication skills.

Preferred Experience - New Graduates Welcome

• Five or more years of family medicine, primary care, urgent care, or comparable outpatient experience is preferred.

• Experience with pediatric and geriatric patients.

• Experience with EHRs, chronic care management, preventive-care metrics, annual wellness visits, and value-based care.

• Experience with point-of-care testing and common office procedures is helpful.

• New graduates with strong clinical preparation, maturity, teamwork, and willingness to receive mentorship are encouraged to apply.

Schedule & Practice Model

• Full-time outpatient position with primary alignment to either Greenhill or Limestone, with reasonable cross-coverage as needed.

• Combination of scheduled primary-care visits, same-day/walk-in care, telehealth, and chronic-disease follow-up.

• Occasional Saturday or extended-hour coverage may be part of the schedule and will be discussed in advance.

• We are building sufficient provider depth so clinicians have backup coverage rather than carrying the practice alone.

• On-site interview with practice leadership and relevant clinical team members.

• For selected finalists, a paid half-day practice experience or skills assessment may be offered after preliminary credential verification so both the candidate and FMAG can evaluate fit.

• No independent patient care or billing is expected before all required employment, licensure, malpractice, credentialing, supervision/collaboration, and payer requirements are satisfied.
