Licensed Practical Nurse

Role Description This full‑time Licensed Practical Nurse (LPN) role is an on‑site position based in Louisville, KY. The LPN provides direct nursing care to individuals served, including monitoring vital signs, administering medications and treatments as ordered, and assisting with activities of daily living. The role includes documenting care accurately, updating medical records, and reporting changes in condition to supervising registered nurses or providers. The LPN collaborates with an interdisciplinary team to support care plans, promote health education, and ensure a safe, respectful environment. Additional responsibilities may include assisting with intake assessments, supporting quality and compliance efforts, and participating in staff meetings and training. Qualifications Current, active Licensed Practical Nurse (LPN) license in the state of Kentucky (or eligibility for licensure) and current CPR/BLS certification. Demonstrated clinical skills in basic nursing care, medication administration, and monitoring of physical and behavioral health status. Strong documentation and electronic health record skills, including accurate, timely charting and adherence to confidentiality standards (e.g., HIPAA). Effective interpersonal, communication, and teamwork skills, with the ability to work respectfully with people from diverse backgrounds and life experiences. Ability to prioritize tasks, follow clinical protocols, and respond calmly in urgent or crisis situations. Experience in community‑based, residential, behavioral health, or social services settings is preferred. High school diploma or equivalent required; completion of an accredited practical nursing program is required. Additional training in mental health, substance use, or community health is a plus.
