You'll be more than a coordinator—you'll be a trusted advocate, problem-solver, and partner in making sure every client receives the right care, at the right time, from the right people.If you believe that people matter, details matter, and doing things exceptionally well matters, we'd love to meet you.What You'll DoAs a Care Coordinator, you'll help ensure every client receives safe, consistent, and compassionate care while supporting our caregivers to succeed.Conduct in-home client assessments and supervisory visits, ensuring care is being delivered safely and according to the client's needs.Develop, implement, and regularly update individualized care plans.Build genuine, trusting relationships with clients, families, caregivers, and referral partners.Listen to clients and families, understand their concerns, and take ownership of finding solutions.Support caregiver onboarding, training, and ongoing supervision to ensure caregivers are equipped to provide exceptional care.Monitor the quality and consistency of care and proactively address concerns before they become larger issues.Ensure compliance with company policies, Arizona regulations, and medical insurance requirements.Maintain accurate, timely, and thorough documentation.Partner closely with Scheduling, Recruiting, HR, and Operations to ensure the entire care team is aligned and supported.Identify opportunities to improve the client and caregiver experience and contribute to continuous quality improvement.Represent TCC with professionalism, compassion, and integrity in every interaction.What We're Looking ForAt least 1 year of experience in care coordination, care management, care supervision, or a related role.Previous experience in home care, caregiving, or community-based healthcare.A genuine passion for helping people and improving the lives of clients and families.Strong communication, relationship-building, and customer service skills.Excellent organization and attention to detail.The ability to manage multiple priorities while remaining calm, responsive, and solutions-focused.Reliable transportation and a clean driving record.Join a Team That CaresIf you're looking for more than a job and want to be part of a team that lives its values every day, Total Care Connections could be the place for you.
