Overview
Physical Therapist – General, Gloversville, NY. Travel - temporary assignment in a nursing home setting. Start date: 11/02/2026. Duration: 12 weeks. Weekly pay: $2,336–$2,466 per week. Shift: 8-hour daytime shifts, Saturdays only, PRN; times 8:00 a.m.–4:30 p.m. Setting: skilled nursing facility. This travel assignment is with Healthforce Healthcare LLC.

Responsibilities
- Conduct PT evaluations for residents and develop individualized plans of care within a SNF setting
- Implement physical therapy interventions including therapeutic exercise, gait training, balance and mobility training, and functional activity work
- Monitor progress, document outcomes, and adjust plans of care in collaboration with the interdisciplinary team
- Maintain accurate records in accordance with facility policies and regulatory requirements
- Coordinate discharge planning and provide patient and caregiver education for home exercise programs
- Ensure patient and staff safety, adhere to infection control practices, and protect patient privacy
- Provide Saturdays-only sessions as PRN per assignment and manage scheduling with supervision and facility needs

Requirements
- Active New York physical therapy license
- BLS certification required
- Prior experience in a Skilled Nursing Facility (SNF) or equivalent long-term care setting required
- Ability to work in a SNF environment and perform Saturday PRN sessions
- Strong documentation and communication skills
- Eligibility to work in the United States
