Surgical Nurse Navigator

Duties

Duties include but are not limited to:

• Participates in the unit/program level quality improvement process and initiatives as well as customer service programs.
• Complies with VA-mandated processes and use of health information solutions that serve to assess, document, exchange, and track care coordination information.
• Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
• Excellent customer service skills, verbal and writing skills.
• Ability to adapt to frequent change, and multi-tasking.
• Surgical Nurse Navigator should possess recent clinical/case management/care coordination/utilization management skills or a combination of all.
• Must be detail oriented, and have problem-solving and researching skills
• Excellent organizational skills and ability to conduct clinical chart reviews, review of medical records.
