Licensed Practical Nurse (LPN)

Position Summary

Licensed Practical Nurse (LPN) responsible for providing general nursing care to patients under the direction of a Registered Nurse and physicians. The role integrates company values into daily practice.

Responsibilities

• Provides skilled nursing care to patients as ordered by the physician and under the supervision of a Registered Nurse.
• Implements orders for medications and treatments as ordered by the physician and in accordance with nursing care plan as written by the RN.
• Provides information to the Registered Nurse to assist in the development of an individualized plan of care for each patient.
• Participates in evaluation of the effectiveness of interventions in relation to patient responses and seeks guidance in redirection when necessary.
• Reports to supervisor on patient care rendered, problems concerning patients’ needs, and resolution to those needs.
• Helps identify patient problems and potential risks and contributes to the development of interventions.
• Assists in identification of patient’s learning needs and reinforces self‑care and other techniques.
• Appropriately designates applicable charges to individual patients using the hospital’s established system.
• May be required to work during inclement weather and other staffing emergencies.
• Provides an environment conducive to safety for patients, visitors, and staff.
• Assesses the risks for safety and implements appropriate precautions.
• Complies with appropriate and approved safety and Infection Prevention standards.
• Maintains regular attendance in keeping with assigned work schedule.
• Performs other duties as assigned to support overall effectiveness of the organization.

Minimum Qualifications

• Two years’ experience in an inpatient hospital setting; inpatient rehabilitation experience preferred.
• Current state license as a Licensed Practical Nurse.
• Current BLS certification.
• Knowledge of nursing theory and practice, infection prevention standards, accreditation standards (TJC, CMS).
• Knowledge of clinical operations and procedures, with proper utilization of policies and guidelines.
• Proficient computer skills including data entry, word processing, email, and records management.
• Critical thinking skills.
• Effective organizational and time‑management skills.
• Effective written and verbal communication skills.
• Ability to prioritize, meet deadlines, and complete complex tasks.
• Ability to maintain quality, safety, and infection prevention standards.
• Ability to work independently.
• Ability to maintain confidentiality.
• Ability to work closely and professionally with others at all levels.

Physical Requirements

• Significant standing, walking, bending, reaching, lifting, pushing, and pulling for prolonged periods.
• Gross and precise motor functions; lifting up to 50 lbs.
• Possible exposure to bodily fluids.
• Visual acuity required for patient assessment and documentation.
• Hearing required for accurate patient assessment.
• Manual dexterity for operating equipment and computer keyboard.
• Close vision and ability to adjust focus.
