Candidates with Hospital or SNF experience will be considered.Experience using electronic medical records. Familiarity with Homecare Homebase preferred.Ability to meet productivity target of 6 points/day (30 points/week).Valid driver's license and reliable transportation for patient visits within a manageable territory.May consider a PT/COTA pair; COTA applicants may be considered when submitted with a PT.Assignment DetailsFacility type: Home Health.Shift name: Days.Schedule options: 5x8 (Monday–Friday) or 4x10s.Primary coverage area includes Northbridge, Upton, Uxbridge, Mendon, Millville, Blackstone, and Bellingham.
