Cancellation Policy: Provider or facility can cancel assignment with a minimum 2 week notice with no penalty. $1,500 cancellation fee without 2 weeks notice, for both providers and facility. Term with cause - no penalty. Facility can cancel 2 shifts per 13 week contract with no penalty - guaranteed hours only apply at 3rd cancellation. Right to hire with no penalty after 13 week assignment completed. Late Cancellation of Shift by Client Affiliate or HealthTrust: 3.11.1 Cancelled during a shift or upon arriving to the Client Affiliate facility for Per Diem Staff – pay Subcontractor for the greater of the number of hours worked or 4 hours. 3.11.2 Cancelled before 12 p.m. Client Affiliate time business day prior to shift for Per Diem Staff - no charge. 3.11.3 Cancelled after 12 p.m. Client Affiliate time business day prior to shift for Per Diem Staff.
