Occupational Therapist

Position Overview

Inabia is  looking for a Occupational Therapist to join our team in a Skilled Nursing Facility (SNF) setting. If you’re passionate about patient-centered care and enjoy collaborating with a supportive team, we’d love to meet you.  You’ll help patients maximize independence with daily activities through evidence-based OT interventions and compassionate coaching.
You’ll collaborate with the care team on goals, discharge planning, and caregiver education to support safe transitions  

Key Responsibilities

• Perform evaluations, develop individualized care plans, and deliver therapy services based on patient needs.
• Provide daily treatments, track functional progress, and support discharge planning.
• Collaborate closely with therapy, nursing, and interdisciplinary teams.
• Complete accurate, timely, and compliant clinical documentation.
• Participate in skilled nursing rehab programming focused on functional recovery and safe transitions.
• Contribute to a team-based approach that prioritizes patient outcomes and quality care.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current state OT license (or ability to obtain prior to assignment).
• Strong communication, time-management, and patient education skills.
• Comfort working independently while partnering with a team.
• Prior SNF experience is a plus (or a strong desire to work in post-acute care).
