Primary Care Nurse Practitioner or Physician Assistant Four-Day Workweek No Call Litchfield, Connecticut

An established adult primary-care practice in scenic northwestern Connecticut is seeking an experienced Nurse Practitioner or Physician Assistant to join its collaborative outpatient team.

This is an excellent opportunity for a provider who wants a predictable four-day schedule, greater ownership of patient care, and the resources of a large, fully integrated nonprofit healthcare system. The selected provider will build and independently manage a continuity panel while receiving structured orientation, clinical support, and access to an extensive network of physicians and specialists.

• Full-time outpatient primary care
• Tuesday through Friday schedule

• No nights, weekends, or call

The Nurse Practitioner or Physician Assistant will provide comprehensive, patient-centered primary care to an established and growing adult population.

Responsibilities include:

• Conducting annual physical examinations and preventive-health visits
• Evaluating and treating acute and chronic medical conditions
• Developing individualized medical, educational, and psychosocial care plans
• Ordering and interpreting laboratory studies, imaging, and diagnostic testing
• Prescribing and managing medications and treatment plans
• Coordinating referrals and collaborating with physicians and specialists
• Educating patients regarding diagnoses, treatment options, preventive care, and lifestyle changes
• Identifying conditions outside the provider s scope and referring appropriately
• Maintaining accurate and timely documentation in Epic
• Independently managing a continuity panel
• Maintaining an average volume of at least 18 patient visits per day

• Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, or Physician Assistant
• Appropriate national certification or board eligibility
• Current or eligible unrestricted Connecticut APRN or Physician Assistant license
• Minimum of two years of experience in traditional primary care or a comparable panel-based outpatient setting
• Demonstrated experience independently managing a patient panel
• Ability to maintain an average volume of at least 18 patient visits per day
• Strong clinical judgment and communication skills
