Outpatient Physical Therapist

Job Description

We are seeking a qualified Outpatient Physical Therapist to evaluate assigned patients and develop, implement, and modify treatment plans according to patient needs, goals, and physician direction. This role is responsible for providing discipline-specific therapy services, ensuring the design of functional treatment programs for effective return to community activities. The selected candidate will also serve as an educator for patients, families, and colleagues and perform additional professional duties associated with patient care and therapy service development.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Develop individualized treatment plans and goals focused on patient discharge progress.
• Document medical records clearly and accurately, in compliance with local regulations and clinic policies.
• Provide therapy under signed, valid physician orders and ensure alignment with insurance approvals.
• Participate in student programs, offering supervision and evaluation to enhance learning experiences.
• Create a professional development plan and pursue continuing education opportunities.
• Adhere to State Practice Act and maintain professional licensure in good standing.
• Engage in performance improvement initiatives to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited program.
• 6 months to 1+ year of outpatient clinical experience preferred.
• Current CPR certification.
• Active and applicable state licensure for physical therapy.
• Previous experience with our agency is a plus.

Schedule

This position operates on a 5x8.0 schedule, starting on August 10th.
