Deficiency Tracking/Supervisor Analyst

Job Summary The Deficiency Tracking/Supervisor Analyst compiles, organizes and analyzes patient medical records for Hospital Inpatient and Hospital Outpatient services to support network goals for accuracy, timeliness and completeness. The role ensures compliance with Centers for Medicare & Medicaid Services, the New Jersey Department of Health, The Joint Commission and Hackensack Meridian Health policies and procedures. Shift EVENING shift: 3 pm – 11:30 pm Monday‑Friday and every other weekend. Responsibilities Retrieve paper records of discharged patients from nursing units on each campus. Ensure all discharged charts are accounted for, using the Chart Reconciliation work queue in Epic. Create a list of all charts not received and communicate with nursing and ancillary staff to resolve status. Organize and assemble charts, removing documents that should not be scanned and verifying correct patient information on each page. Prepare medical records for scanning, labeling documents and removing staples and tape. Scan documents into the legal medical record, ensuring complete originals are scanned appropriately for data integrity. Perform quality control of scanned images for accuracy; rescan and reindex originals as needed. Investigate, analyze, and verify patient information using multiple hospital computer applications to perform document corrections. Assist with medical record merging of duplicate potential merges as per departmental policy. Accurately review and analyze HI deficiencies in Epic with a 98% accuracy rate, selecting responsible physicians. Re‑analyze records to ensure information meets regulatory content requirements. Monitor and review Chart Deficiency work queues for timeliness and accuracy. Perform physician suspension process by notifying providers on delinquent records. Assist physicians in completing their medical record deficiencies. Process release of information requests as necessary. Assist patients with MyChart issues, providing verification codes, password assistance, and proxy access. Request archived medical records from the storage vendor on a timely basis. Answer the telephone within three rings, identifying self and department. Respond professionally to callers and address their needs appropriately. Route medical record requests received to the ROI vendor. Advise the Coordinator or Supervisor on situations requiring follow‑up. Meet specific performance metrics of productivity, accuracy, and quality assurance. Perform other duties and projects as assigned. Adhere to HMH Organizational competencies and standards of behavior. Qualifications Education, Knowledge, Skills and Abilities Required: High School diploma, general equivalency diploma (GED), or equivalent. Good organizational and communication skills. Computer literate; proficient with Microsoft Office and Google Suite. Excellent written and verbal communication skills. Ability to bend, lift and carry up to 10 lbs for extended periods. Ability to walk and/or travel as needed to different floors, buildings, and sites within the HMH network to retrieve documentation. Education, Knowledge, Skills and Abilities Preferred: Experience in health information, health care, or a related field. Compensation Minimum rate: $21.73 per hour.
