Description

• Conduct comprehensive evaluations of patients in both inpatient and outpatient settings while crafting personalized treatment plans tailored to their unique needs
• Implement physical therapy methods emphasizing mobility enhancement, strength development, balance improvement, and functional rehabilitation
• Collaborate closely with nurses, physicians, case managers, and a diverse range of rehabilitation professionals to elevate patient outcomes
• Guide patients and their families through therapeutic objectives, discharge strategies, and home exercise regimens
• Maintain thorough documentation of evaluations, treatment sessions, and patient progress in alignment with facility and regulatory norms
• Engage in care team meetings and interdisciplinary conferences to establish seamless care coordination

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Current state licensure as a PT
• CPR certification is preferred
