Job Summary
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our evening shift team at Desert Peak Care Center in Phoenix, Arizona. As an LPN at our care center, you will play a vital role in providing high-quality care to our residents while promoting a positive and supportive environment. Responsibilities
Provide direct patient care, including administering medications, wound care, and vital sign checks
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Communicate effectively with residents, families, and healthcare staff to ensure seamless care transitions
Participate in quality improvement initiatives and maintain accurate and timely documentation
Collaborate with the healthcare team to identify and address resident needs and concerns
Maintain a clean and organized work environment that promotes patient safety and comfort
