As our LPN Admission Nurse , you'll play a key role in helping older adults and their families navigate one of life's most important transitions with confidence, compassion, and dignity. This isn't a traditional floor nursing role. You'll be the clinical coordinator behind seamless admissions, working closely with residents, families, providers, and interdisciplinary teams to ensure every new resident receives a safe, organized, and welcoming start in our Residential Care community. What You'll Do As the primary clinical contact for admissions, you'll: Coordinate new admissions, readmissions, and short-stay resident transitions. Review clinical referrals, medical records, and admission documentation. Complete nursing assessments and support person-centered care planning. Conduct medication and treatment order reviews and assist with medication reconciliation. Partner with residents, families, physicians, hospitals, therapists, and community providers. Ensure all admission documentation is accurate, complete, and compliant. Help residents and families feel informed, supported, and welcomed. Collaborate with nursing, caregiving, therapy, and administrative teams to create smooth transitions into care. Support continuity of care for Friendsview Life Plan members utilizing short-stay or recovery services. Follow up with new residents to ensure a positive admission experience. What We're Looking For Required Qualifications Current CPR/BLS certification (or ability to obtain) Strong clinical assessment and documentation skills. Excellent communication and organizational abilities. Ability to manage multiple priorities while maintaining attention to detail. Commitment to person-centered care and exceptional customer service. Preferred Qualifications Experience in assisted living, residential care, skilled nursing, rehabilitation, home health, hospice, or senior living. Experience with admissions, transitions of care, discharge planning, or medication reconciliation. Familiarity with electronic health records (EHRs).
