Care Manager: Community Outreach & Client Advocate
Workstream
Workstream is seeking a Care Management & Professional Outreach specialist in North Carolina. The role involves working directly with clients and their families to develop personalized care plans and actively participating in community outreach efforts. The ideal candidate will hold at least a Bachelor’s Degree in a healthcare-related field, possess effective communication skills, and have a minimum of two years experience in care management.
