Job responsibilities may be modified or expanded over time, and the company will inform the employee of any such changes.\n\nMULTI-EMPLOYER ROLE\nThis W-2 hourly Family Nurse Practitioner position includes responsibilities that span across the following three organizations, all under shared leadership:\n\n FOCUS #1 Community Wellness Outpatient Mental Health Center\n FOCUS #2 Lighthouse Behavioral Health Center\n FOCUS #3 Freedom Healthcare\n\nYou will maintain separate W-2 employment relationships with each organization and will receive individual payroll compensation from each entity based on the time worked and services provided for their respective programs. This means you will be an official employee of all three companies, with hours, responsibilities, and compliance obligations tracked separately for each.\n\nThis role is structured as a 24-hour per week position, with approximately one-third of the total weekly hours (about 8 hours per week) allocated to each organization. While day-to-day supervision and scheduling will be coordinated centrally to support integrated care, each employer will manage its own onboarding, HR, and compliance processes in accordance with applicable laws and organizational policies.\nDIVISION: Operations\nACCOUNTABLE TO: Operations Manager\nCLASSIFICATION\nThis is a part-time, W-2 employee position totaling 24 hours per week, worked over three scheduled days, with hours allocated across three affiliated companies as follows:\n\nFreedom Health Care – Freestanding General Primary Care Clinic (FHFSGC):Part-time, W-2 employee — approximately 2.4 hours per week (10% of total weekly hours)\nCommunity Wellness Outpatient Mental Health Center (CWOMHC):Part-time, W-2 employee — approximately 10.8 hours per week (45% of total weekly hours)\nLighthouse Behavioral Health Center (LHBHC):Part-time, W-2 employee — approximately 10.8 hours per week (45% of total weekly hours)\n\nThis role is one of five individuals collectively assigned to meet and exceed the COMAR-mandated leadership coverage requirements, which include:\n\n20 hours per week for a Program Director at Community Wellness OMHC\n20 hours per week for a Medical Director at Community Wellness OMHC\n20 hours per week for a Clinical Director at Lighthouse Behavioral Health Center\n\nCollectively, the five assigned professionals provide a total of 120 hours of leadership coverage per week, which exceeds the COMAR requirements for each organization as follows:\n\nCommunity Wellness OMHC (Backup Program Director Requirement):COMAR requires 20 hours/week; the team provides 24 hours/week→ Exceeds requirement by 4 hours/week\nCommunity Wellness OMHC (Medical Director Requirement):COMAR requires 20 hours/week; the team provides 54 hours/week→ Exceeds requirement by 34 hours/week\nLighthouse Behavioral Health Center (Clinical Director Requirement):COMAR requires 20 hours/week; the team provides 42 hours/week→ Exceeds requirement by 22 hours/week\n\nThis structure ensures robust clinical leadership and regulatory compliance across all entities, with ample coverage to support oversight, team collaboration, and quality improvement initiatives.\nWORK SCHEDULE\nThe Psychiatric Mental Health Nurse Practitioner (PMHNP) will work every Monday, Friday and one other set weekday each week, from 8:00 AM to 5:00 PM on each assigned day. The schedule will include one full administrative day (Fridays) and two days (Thursday and one other set weekday) focused primarily on direct client care and clinical services.\n\nFriday of each week is designated as the Administrative Day, with structured responsibilities that include leadership meetings, documentation audits, quality reviews (COMAR, CARF, policies), psychiatric updates (90792), and team collaboration.\nThe Friday and one other set weekdays will focus on the provision of psychiatric services, including:\n\nPsychiatric focused evaluations and medication management\nSubstance use disorder focused evaluations and medication management\nPrimary care focused evaluations and medication management\nCare coordination and supervision of multidisciplinary clinical staff\nDocumentation, prescribing, treatment planning, and compliance with CWOMHC / LHBHC / FHFGC service standards \n\nCOMPENSATION RANGECompensation for this position ranges from $30.00 to $100.00 per hour, and is commensurate with experience, expertise, verified credentials, and available company budget.\nIn addition to hourly wages, eligible employees may receive a comprehensive benefits package that includes:• Paid Time Off (PTO)• Family and Medical Leave• Health, Medical, and Dental Insurance Reimbursement or health insurance coverage, as available• Supplemental Health and Disability Insurance Options• Retirement Savings Plan• Professional Development Support and Continuing Education Opportunities ANTICIPATED TRAVEL: 5% of the time. There is no additional compensation for travel. DUTIES AND RESPONSIBILITIES1. Facilitate and document coordination with external primary care providers, as clinically appropriate and with proper consent.2. Prescribe medications following a full face-to-face evaluation and assess for any contraindications prior to initiating a medication regimen.3. Educate clients and, when appropriate, caregivers on the purpose, effects, and risks of prescribed medications.4. Monitor and adjust medication regimens as clinically indicated; conduct evaluations at a minimum every 90 days for ongoing medication management.5. Notify the client's primary care provider of new prescriptions or medication changes every six months or as clinically required.6. Conduct peer reviews of other prescribers’ work and support documentation compliance across the clinical team.7. Participate in interdisciplinary team meetings with HR, Scheduling, Patient Access, and other departments to support clinical and administrative needs.8. Conduct batch documentation audits, including review and signature of referrals, treatment plans, biopsychosocial, ASAMs, and intern notes.9. Collaborate with external agencies or treatment providers in cases involving shared clients or complex care needs.10. Accept and review incoming lab results in ICANotes; use clinical judgment to adjust care plans accordingly.11. Ensure all documentation is completed within expected timeframes using ICANotes and is coded appropriately using evaluation and management CPT codes.12. Contribute to quality assurance, audit readiness, and performance improvement initiatives to ensure compliance with COMAR, CARF, Medicaid, and internal standards.13. Provide support and education to clinical staff related to medication protocols and service delivery expectations.14. Submit periodic reports and summaries related to administrative and clinical oversight duties.\n15. Administrative Leadership – Friday (8:00 AM – 5:00 PM)\nFridays are fully dedicated to fulfilling administrative and clinical leadership responsibilities across all three affiliated programs:\n\nCommunity Wellness Outpatient Mental Health Center (OMHC)\nLighthouse Behavioral Health Center\nFreedom Healthcare – Community Wellness Primary Care Clinic\n\nThe PMHNP will support clinical compliance, supervision, documentation review, quality assurance, and coordination with executive and program leadership in alignment with all applicable COMAR regulations, CARF standards, and internal policies.\nThe schedule for this dedicated administrative day includes:\n\n8:00 AM – 9:00 AMParticipate in the weekly leadership meeting, including:\n\nPatient Access Specialists 1 & 3\nScheduling Department Supervisor\nPRP/BHH Director\nHousing Services Director\nCOO and CEO\n\n9:00 AM – 9:15 AM\n\nPlan the day with the Operations Manager\nConduct administrative tasks (email, voicemail, calls)\nMeet with administrative assistant or conduct scheduled HR interviews\nDocument care coordination using appropriate CPT codes (99211–99215)\n\n9:15 AM – 9:30 AMShadow one random session conducted by a Clinical Supervisor or Prescriber for quality and mentorship purposes\n9:30 AM – 10:00 AMParticipate in the weekly budget vs. actual financial review with:\n\nFinance Division Manager\nAccounts Receivable and Accounts Payable Managers\nCOO\nFocus: SUD OP, IOP, and Residential Programs\n\n10:00 AM – 10:45 AMConduct weekly clinical supervision with the organization’s Clinical Director (or substitute with HR interview if applicable)\n10:45 AM – 11:00 AMSelf-directed administrative time, continued communication follow-ups, and care coordination documentation (CPT 99211–99215 as applicable)\n11:00 AM – 11:45 AMConduct monthly individual clinical supervision with:\n\nOne prescriber (from pool #23, 36, 40, 45, 79, 89, 91, or 95) not yet seen this month\nOne Registered Nurse or Somatic Nurse Practitioner\nUse Quick Notes supervision code 11111 to log each session\n\n11:45 AM – 12:45 PMLunch\n12:45 PM – 1:15 PMParticipate in the OMHC and FGC budget vs. actual financial meeting with:\n\nFinance Division Manager\nAccounts Receivable and Accounts Payable Managers\nCOO\n\n1:15 PM – 2:15 PMHost the weekly all-staff meeting or in-service for OMHC/SUD counselors and therapists as scheduled\n2:15 PM – 4:45 PMConduct five 90792 psychiatric evaluation update sessions with clients who have not been seen by the Medical Director in the past 180 days\n4:45 PM – 5:00 PMSubmit a Daily Report-Out to the Operations Manager summarizing:\n\nClinical supervision completed\nFinancial review highlights\nStaff meetings or consultations held\n90792 evaluations conducted\nOutstanding administrative follow-ups\n\n16. Clinical Days – Mondays and one other set Weekday (8:00 AM – 5:00 PM)\nDuring clinical days, the PMHNP will provide psychiatric care, medication-assisted treatment (MAT), and integrated primary care coordination across the following affiliated programs:\n\nCommunity Wellness Outpatient Mental Health Center (CWOMHC)\nLighthouse Behavioral Health Center\nFreedom Healthcare – Community Wellness Primary Care Clinic (a freestanding general clinic)\n\nThe schedule for these dedicated clinical days include:\n\nConducting psychiatric evaluations, re-evaluations, and follow-up medication management visits for individuals receiving outpatient mental health, substance use disorder, or primary care services\nDelivering Medication Assisted Treatment (MAT) using FDA-approved medications such as buprenorphine, in alignment with COMAR 10.09.80 and federal MAT standards\nResponding to medical emergencies in outpatient or residential care settings and conducting diagnostic assessments as clinically indicated\nManaging and documenting all services in the electronic health record (EHR) using appropriate billing codes, including:\n\n90791, 90792, 99202–99205, 99211–99215, H0004, H0032\n\nCoordinating care across multidisciplinary teams and external providers to ensure continuity of care and comprehensive support for physical and behavioral health needs\nServing as the Supervising Provider on Duty during all scheduled shifts for both CWOMHC and Lighthouse Behavioral Health Center, providing clinical oversight for therapists, nurses, prescribers, and support staff\nReferring clients to the Psychiatric Rehabilitation Program (PRP) when clinically appropriate and completing necessary documentation and care transition planning\nSupporting and, when assigned, delivering basic primary care services at the Freedom Healthcare freestanding general clinic, including chronic disease management, health screenings, and coordination with behavioral health providers\nEnsuring that all services adhere to the standards outlined in COMAR 10.21.20, 10.09.80, 10.47.01, and any applicable state and federal healthcare regulations\n\nPHYSICAL DEMANDS: Regularly walk, stand or stoop; occasionally lift, carry, push, pull or otherwise move objects weighing up to 25 pounds; and regularly drive a motor vehicle. WORKING CONDITIONS: All work is performed in an office where staff is exposed to communicable diseases of patients. COMPETENCIES AND SKILLS1. Provision of credentialed focused evaluations, treatments and assessments to patients2. Monitor and evaluate individual treatment plans and service modalities rendered by staff under your oversight with the goal of enhancing effectiveness, streamlining implementation, assuring quality and model fidelity3. Prescribe and manage medications for patients with focus on somatic/chronic conditions4. Monitor and evaluate the effectiveness of treatment plans5. Collaborate with other healthcare professionals to provide comprehensive care6.
