Hybrid Case Manager - Veterans Housing & Support

Kaizen Lab Inc.

Community Hope is seeking a full-time Case Manager for our Bringing Veterans Home program in a hybrid role. You will provide case management, eligibility screening, and linkage to housing, financial assistance, and supportive services for veterans in need. The position emphasizes outreach, documentation, and collaboration with community and governmental agencies. A Bachelor's degree and 1 year of relevant experience, plus a valid driver's license, are required. #J-18808-Ljbffr Kaizen Lab Inc.
