Case Management & Social Work Coordinator

Wexner Medical Center is seeking a Case Management Extender to provide essential programmatic support for Case Management and Social Work at The James Cancer Hospital. The role includes coordinating program support, implementing discharge plans, and scheduling. The ideal candidate will hold a Bachelor’s degree, preferably have experience in an Acute Care setting, and possess strong communication and organizational skills.
