Key Responsibilities

• Conduct skilled home health visits for a diverse patient population including med/surg, cardiac, ortho, post-surgical, neurological, respiratory, and other complex cases
• Perform and document OASIS assessments in accordance with Medicare, Medicare Advantage, and commercial insurance requirements
• Complete 6-8 home visits per day, meeting a minimum of 6 points per day as directed by management
• Self-schedule visits and hours based on the needs of the home care office
• Utilize facility-provided laptop, MiFi, and portable printer for documentation using designated scheduling and timesheet systems
• Availability for 1-2 weekend shifts per contract period

• Active Florida or Compact Occupational Therapist license required at time of submission
• Basic Life Support (BLS) certification required
• Minimum 1 year of occupational therapy experience required
• Home health experience required
• Valid driver's license and personal vehicle required to conduct home visits
• Must be COVID-19 vaccinated or have an approved exemption on file; flu shot or declination required (mandatory mask wearing applies in either case)
• Must reside more than 100 miles from the facility to be eligible

• OASIS training or prior OASIS documentation experience
• Experience with Cerner electronic health records
• Familiarity with skilled home care documentation and Medicare/Medicare Advantage billing requirements

• Work autonomously in a field-based setting serving a clinically diverse patient population across a wide range of diagnoses and conditions
• Equipment including a laptop, MiFi device, and portable printer is provided to support efficient documentation in the field
