Oak Hill TGH - Program Coordinator

Purpose Statement Responsible for the delivery and implementation of behavioral health or recovery programs within a facility. Essential Functions Provide patient care to include coordinating activities, providing companionship and assisting with basic daily needs (dressing, bathing, eating). Collaborate with treatment team members to assist with patient treatment and/or care goals. May serve as a co-lead for groups. Monitor the daily well-being of patients and identify need for social intervention. Ensure all documentation regarding the program, such as care, treatment or incidents, is completed in a clear, concise, professional and completed in a timely manner. Demonstrate a positive, empathetic and professional attitude towards customers always. When patient needs are not met, acknowledge and work to resolve complaints. Recognize that patient safety is a top priority. Other Functions Perform other functions and tasks as assigned. Education / Experience / Skill Requirements Bachelor's degree in Human Services, Social Work or related field required. One or more years previous experience in behavioral health with population of the facility preferred. Licenses / Designations / Certifications Certification in specialty of program preferred. CPR and de-escalation/restraint certification required (training available upon hire and offered by facility). First aid may be required based on state or facility.
