Travel Registered Nurse (RN) – Home Health

Location: La Grange, KY (Coverage includes Jefferson, Oldham, Henry, Trimble, and Carroll Counties)

Pay: $58.00–$59.21 per hour

Position Type: Short-Term Contract

Schedule: Monday–Friday | 8:00 AM – 4:30 PM

Weekly Guarantee: 32 hours

Mileage Reimbursement: Yes – reimbursed between patient visits (from first patient to last patient)

Job Responsibilities

• Deliver skilled nursing care to patients in their homes.

• Complete comprehensive patient assessments and documentation.

• Accurately complete OASIS assessments.

• Develop, implement, and evaluate individualized patient care plans.

• Educate patients and caregivers on disease management, medications, and treatment plans.

• Collaborate with physicians and interdisciplinary healthcare teams to ensure quality patient outcomes.

• Maintain accurate and timely electronic documentation.

• Travel throughout assigned coverage areas to provide patient care.

Schedule & Coverage

• Monday–Friday

• 8:00 AM – 4:30 PM

• 32 guaranteed hours per week

• On-call: One weekday each week and one weekend per month

• Coverage includes:

• Jefferson County

• Oldham County

• Henry County

• Trimble County

• Carroll County

Qualifications

• Active Registered Nurse (RN) license required.

• Minimum 1 year of nursing experience required.

• Experience in Home Health preferred.

• Ability to work independently while providing compassionate, patient-centered care.

• Excellent assessment, communication, and documentation skills.

Preferred Qualifications

Candidates with the following experience are highly encouraged to apply:

• Homecare Homebase (HCHB) experience

• OASIS assessment experience

• Strong hands-on clinical nursing skills

• Previous home health nursing experience

Pay Details: $58.00 to $59.21 per hour
