Your responsibilities include evaluating patient mobility, developing individualized treatment plans, providing therapeutic exercises, gait training, and mobility education, and documenting care in Homecare Homebase EMR. You will cover Glendale, Peoria, Sun City, and Surprise, and receive mileage reimbursement at $0.72 per mile
