Registered Nurse - Hospice

Registered Nurse – Hospice – Morristown, NJ 07920 The Registered Nurse – Hospice will be working for a Major Healthcare System and has career growth potential. Highlights Shift Options: Monday–Friday, 8:00 AM–4:00 PM; on-call/weekends required (1 weekend per month) 11a-7p and every other weekend Pay Range: $60–65 hourly, plus mileage reimbursement for travel to different counties to visit patients as needed Responsibilities Perform patient assessments to determine appropriateness for hospice care; identify learning needs and progress toward goals Develop, implement, and revise patient/family plan of care; prioritize care needs Complete admission/discharge assessments and documentation within required timeframes; transfer data twice daily Provide age‑appropriate teaching to patients/families and document education provided Provide case management including interdisciplinary communication and participation in IDT meetings (every 2 weeks) Conduct HHA supervisions per standards; follow scheduling process and productivity standards Maintain safe environment; follow infection control practicesMedication management, including routine medication reconciliation and maintaining current medication orders Participate in performance improvement/UR activities as requested; support orientation/mentorship and cross‑training Qualifications Current New Jersey RN License (ASN or Diploma accepted) BLS required Valid New Jersey driver’s license required 2+ years RN experience in a home hospice environment (3+ years preferred) EMR experience; strong critical thinking and communication skills #J-18808-Ljbffr
