LPN Intake Coordinator

LPN Intake Coordinator Key Responsibilities. a. Supervision and Management: • Supervise, mentor, and evaluate a team of LPN in home health to ensure effective skilled nursing practices. • Conduct regular team meetings and one-on-one sessions to provide feedback, support, and development opportunities for LPN nurses in the home health setting. Offer support through frequent communication with nurses within their caseload. • Ensure that case management activities are conducted in accordance with organizational policies and regulatory requirements. • Provide training and professional development opportunities to nurses to enhance their skills and knowledge. b. Patient Care Coordination: • Oversee the development and implementation of individualized care plans for patients. Review care plans with RN supervisor for final sign off. • Collaborate with healthcare providers, patients, and families to ensure seamless coordination of care. • Ensure that patients receive timely and appropriate services, including medical care, rehabilitation, and social support. • Address and resolve any patient care issues or concerns that arise and follow with chain of nursing leadership to help address any withstanding issue. • Assist in growing business operations and reaching target revenue and growth goals for the Nursing Department with outreach and recruitment practices. • Coordinate and facilitate family/nurse and travel to individuals' home for meet and greets, intake meetings, and recertification assessments. c. Compliance and Quality Assurance: • Ensure that all case management activities comply with relevant laws, regulations, and accreditation standards. • Partner with the Nursing Operations team to conduct regular audits of case management records and processes to ensure quality and compliance. • Implement quality improvement initiatives to enhance the effectiveness of case management services. • Stay informed about changes in healthcare regulations and best practices related to case management. d. Communication and Collaboration: • Serve as a liaison between the nursing case management team and other departments within the organization. • Foster and build strong working relationships with healthcare providers, community organizations, support coordinator organizations and external partners. • Communicate effectively with patients, families, and healthcare providers to ensure that care plans are understood and followed. e. Administrative Duties: • Manage the scheduling, payroll, and administrative tasks related to the nursing team. • Prepare reports on nurses' performance, patient outcomes, and other relevant metrics.
