Emplify Health is seeking a dedicated and flexible Physical Therapist to join our on-site team serving the Green-Bay area from our hospital-outpatient clinic. In this full-time role, you'll work closely with primary care providers to ensure improved health outcomes through integrated, patient-centered care working with patients of diverse backgrounds, approximately 30% of which are Spanish-speaking.

Key Responsibilities:

• Collaborate with primary care teams to develop and implement care plans.
• Perform physical therapy consultations in a primary care setting.
• Provide compassionate care to patients from diverse backgrounds.

Qualifications:

• Autonomous and adaptable with strong communication skills.
• Experience working with diverse populations; Spanish proficiency or familiarity with interpreter services is a strong plus.
• Ability to thrive in a collaborative, fast-paced healthcare environment

- Our team of therapists provide care in the following areas:

- Outpatient orthopedics in both primary care and specialty clinics

- We have a rehabilitation team consisting of physical therapists, occupational therapists, and speech therapists. Therapists who are still embedded in patient care also lead our teams.
