Job Description Care2U is a fast-growing healthcare startup providing on-demand acute care and hospital-level care in the patient's own home throughout New York City, Nassau, and Westchester counties. Our mission is to reduce unnecessary ED visits and hospitalizations by delivering high-quality care where patients feel most comfortable — at home. This is a remote telehealth position. However, initial training will be conducted in person in Plainview, NY. Candidates must hold a valid New York medical license. Job Responsibilities: Conduct patient assessments via telehealth with the assistance of on scene PAs, NPs or Paramedics to manage patients with acute illnesses and injuries. Provide treatment and clinical management consistent with evidence-based guidelines. Respond to on-demand and scheduled calls and focus on quality and efficiency metrics to improve patient experience. Work collaboratively with our multi-disciplinary team of field providers and care coordinators to efficiently manage complex care needs in the home. Assess patients for risk factors associated with the care of the acute and complex chronically ill patient including medication compliance and side effects, limited mobility, impaired nutrition, fall safety, and social isolation. Complete patient records in the Electronic Health Record in a timely manner that meets acute care compliance standards and captures patient acuity. Maintain a safe, secure, equitable and healthy work environment by following safety procedures, quality standards and comply with regulations. Stay current on in-home acute care literature, technology, and regulatory issues. Job Type: Full-Time Total comp: >>$300k.
