JOB PURPOSE/SUMMARY

Summary of role of team :

The SOR Recovery Life Coach will work as member of a multi-disciplinary team within the Center's outpatient SUD services continuum to provide engagement and recovery support to clients, family members and other parties affected by substance use and co-occurring disorders.

Summary of position :

The SOR Recovery Life Coach works compassionately with individuals to provide hope, encouragement, support, and connection to needed services.

The recovery coach interacts with internal and community partners in a professional way ensuring positive two-way communication for the benefit of those we serve.

Helping individuals meet employment, educational, life skill and other goals is essential to the position.

Engages with patients in medication-assisted treatment programs and encourages them to maintain recovery, participate in planned recovery-oriented activities and develop a network of recovery supports.

Participates in treatment activities and models effective group participation and recovery-oriented perspective for patients with substance use disorders.

Helps patients identify and access needed resources and recovery supports.

TYPICAL WORKING CONDITIONS/ENVIRONMENT

This position will take place in a combination of office and community settings; may include community visits/transportation to community resources and services.

JOB DUTIES/RESPONSIBILITIES

1. Quality Care

Provides face-to-face engagement, non-judgmental support and recovery coaching for clients experiencing substance use and/or co-occurring disorders who are seeking treatment and recovery resources.

Assists clients in identifying and resolving personal and environmental obstacles to recovery, such as employment, health, social services, and recovery support.

Offers 1:1 Recovery Coaching utilizing motivational interviewing and other evidenced-based practices.

Provides primary coverage and screening services for Center's walk-in/open-access process (screening, assessment and navigation into services).

Updates and maintains the Center's community resource directory.

Responsible for maintaining the Center's presence on the TNRecover App, including app administration responsibilities and the client enrollment/engagement process.

Plans and facilitates frequent, on-going recovery support groups and pro-social activities for clients to practice their recovery skills in a safe, supportive environment.

Keeps up-to-date referral information and contacts with internal and external resources.

Participates in staff, community and program meetings.

Responds to all messages, emails, and voicemails within 24 hours.

When needed, assists program clinical staff in facilitating case management services between the primary care provider, behavioral health provider, pharmacy, specialists, and/or natural supports regarding health and recovery needs on behalf of the client.

Provides coverage for caseload coverage to ensure services continue when on leave

Provides crisis intervention and emergency services as needed.

2. Documentation Requirements

Completes all required clinical documentation within designated timeframes and in accordance with Center Policy and Procedure and funding source guidelines.

Completes client screening tools and enters all client session notes/data into the local/state/federal database/EMR.

Documents all program activities as directed in the format and within the timeframes required.

When needed, assists clinical services team to ensure that all clients have an up to date ROI, care plan, crisis plan, consent to contact after discharge, DLA-20, and updated consents and signature admissions page every 6 months and/or as needed.

Keeps all private and/or confidential information in secure location with a locked file cabinet or in approved locked HRMC bags.

Assists supervisor with data, scores, measurements and other information as appropriate with the goal of outcome reporting, Quality Assurance measurement and stakeholder satisfaction.

3. Team Atmosphere and Professionalism

Interacts professionally with other employees, clients, and community organizations to promote harmonious relationships and attitudes.

Sets and maintains appropriate boundaries for quality service delivery and healthy self-care.

Maintains healthy two-way communication in person, by telephone and within writing to effectively serve clients.

Works independently while understanding the necessity for communicating and coordinating work efforts with other employees and organizations.

Maintains regular/predictable attendance and is punctual to meetings, client sessions and other Center-related activities.

Attends and participates in supervision, administrative/staff team meetings and weekly multidisciplinary treatment team meetings, without tardiness.

Demonstrates flexibility and accepts additional assignments and/or changes in assignments to meet the needs of the SUD Treatment and Recovery Services division.

Communicates and approaches conflict w/ solution-focused approach seeking a win/win outcome.

Presents in a professional manner and adheres to appropriate boundaries in each setting, acting as a positive representative of the team, agency and community.

Promotes an environment in which the culture and beliefs of the individual are respected.

Adheres to the Center's & CPRS policies, procedures and Code of Ethics.

Adheres to the following call-out procedure: if an employee is going to be absent, that employee must notify the supervisor via phone or text before 8 am the day of absence. In addition, if an employee has client responsibilities, this coverage will be arranged with the supervisor during the call-in procedure.

COMPENSATION:

Starting salary for this position is approximately $19.54 /hr based on relevant experience and education.

Schedule:

Regular, timely attendance on shifts is an essential job function.

Position is full-time, 40 hours per week with Center recognized holidays off.

Worked hours will vary; the majority of time will be scheduled Monday-Friday between 8:00am and 5:00pm, with the possibility of evening or alternate hours 3-4x per month for facilitation of client recovery activities.

Travel :

This position will require utilizing a personal dependable vehicle to conduct Center business.

Maintaining a dependable vehicle and certified driver status is a condition of employment.

Staff members may be required to provide transportation in their personal vehicle and staff members are reimbursed for use of their vehicle for this purpose.

Equipment/Technical Competency :

Computer skills/experience and the ability to navigate multiple platforms (TNWITS , EMR, Relias, and Microsoft Office Word/Excel/Outlook) is necessary for timekeeping, training modules, scheduling and use of electronic medical records entry.

Equipment/Technology:

This position does require computer skills and familiarity with Microsoft Office products for timekeeping, scheduling, maintaining the TN Recover App, and documentation in the Center's electronic medical record and the State's database for grants.

QUALIFICATIONS - SOR Recovery Life Coach

Experience / Knowledge:

Must have coursework and/or experience and understanding in the areas of cultural diversity, human development, and treatment of substance use disorder, understanding of trauma-informed care, suicide prevention, crisis intervention referral and knowledge of overdose reversal.

Experience working in a community, residential treatment and/or hospital setting and strong knowledge of community resources and referrals is preferred.

Experience / Education :

Must have a minimum of a bachelor's degree in psychology, social work or related field or a bachelor's degree in another field with previous experience related to the specific duties of the position and the field of substance use treatment.

Lived experience is preferred, but it is not required.

Must possess excellent interpersonal skills and the ability to form meaningful relationships with individuals in or seeking recovery and in working with other professionals (internal and external to McNabb) who serve this population.

Must possess and maintain boundaries appropriate to the setting while adhering to privacy, confidentiality and ethics related to the field.

Physical/Emotional/Social -Skills/Abilities:

Exposure to biological hazards.

Hearing of normal and soft tones.

Lifting up to 50 lbs.

Pushing/pulling up to 150 lbs.

Frequent sitting, standing, walking, bending, stooping, and reaching.

Must maintain up-to-date training required to be certified in, and effectively implement, CPPR and Handle with Care (verbal and physical de-escalation techniques) that include a wide range of bodily movements; including but not limited to, grasping, holding another person, running, and walking.

Must possess a valid TN Driver's license, obtain an F endorsement upon hire and be able to utilize personal vehicle in the exercise of Center job duties.
