Care Manager LCSW at Providence Little Company of Mary Medical Center San Pedro in San Pedro, CA. This position is Full time and will work 8‑hour day shifts. Care Management within the Department of Psychiatry includes a collaborative practice with patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community. Social workers facilitate communication and care coordination along a continuum through effective transitional care management, recognizing the patient's right to self‑determination and the complexities of care such as barriers to coping, abuse, mental health, substance use and other psychosocial issues. Responsibilities Facilitate communication and care coordination across a continuum of care, ensuring effective transitional care management and honoring patients’ rights to self‑determination. Required Qualifications Master's Degree in Social Work. Coursework/Training: All LCSWs must follow the guideline by Los Angeles County Department of Mental Health for LPS designation and maintain current status. Upon hire: California Clinical Social Worker License. Within 18 months of hire: Designated as Behavioral Health Clinician to perform 5150 evaluations. Within 1 year of hire: CPI certification (nonviolent crisis intervention training). 2 years of healthcare‑related setting with psychiatric mental health patient population, preferably in an inpatient setting. Computer experience. Preferred Qualifications Discharge planning experience.
