RN Care Manager - GBMA

Overview The RN Care Manager is responsible for comprehensive quality care management and coordination of clinical activities for primary care patients. The role focuses on early intervention for complex patients, co‑developing care plans to prevent complications and negative outcomes, and promoting population management through education, self‑management support, and timely health care delivery. Education & Experience Registered Nurse license required. Bachelor of Science in Nursing preferred. 1‑2 years of clinical experience preferred; not required. Extensive knowledge of disease management, diagnostics, treatment, prognosis, community resources, and levels of care. Preferred experience in: coordination of community resources, care management of diverse patient populations, ambulatory care practices. Working knowledge of Maryland State Care Management models. Skills Clinical assessment, interviewing and patient education skills. Data collection, analysis and reporting for program evaluation. Strong oral and written communication to address inter‑ and intradepartmental concerns, solve problems, and manage conflict. Innovative problem‑solving using available resources. Computer and personal productivity skills. Commitment to continuing education and professional activities. Compassionate and empathetic demeanor with a can‑do attitude. Responsibilities Assess patients within a defined caseload to identify clinical/medical needs, financial resources, and care goals. Continuously assess care management needs through frequent contact with the care team, patient, and family. Actively manage assigned panel of chronic care patients (high acuity/complex) by developing relationships and ensuring compliance with recommendations (medications, labs, specialist visits, PCP visits, dietitians, CDE, etc.). Execute standing orders for tests and preventive services. Manage referrals to specialists, hospitalizations, ER visits, ancillary testing, and other services. Be available to provide telephone advice per protocol, and handle urgent and emergent calls. Prioritize care management activities to achieve optimum quality and cost outcomes. Coordinate referrals and transitions in care with provider, care coordinator, behavioral health care manager, team members, and payer resources. Develop and implement plans of assessment, education, goal setting, and outcome tracking. Report at APC meetings and provide quality metrics (patient satisfaction, etc.). Participate in monthly provider meetings and practice strategy for performance improvement. Collaborate with clinical teams to pre‑visit planning, after‑visit summaries, patient engagement, and self‑management education. Work with MIS team to facilitate registry reporting and documentation of evidence‑based guidelines. Coordinate change readiness and individualized care plans; assist patients in setting SMART goals. Assess barriers, link to alternate resources, and oversee development of patient self‑management educational materials. Collaborate with payer case managers for additional services where appropriate. Compensation Pay Range: \$68,281.18 - \$110,274.20 (final salary offer will be based on qualifications, education, experience, and alignment with organizational needs).
