Shift Details: 07:00 PM - 07:00 AM, 3 shifts per week, 36 scheduled hours.

Job Order Details: Start date 08/02/2026, end date 10/31/2026, duration 13 weeks.

Client Details: City Sierra Vista, State AZ, Zip Code 85635, Trauma Level Level 3.

Cancellation Policy: Provider or facility can cancel assignment with a minimum 2 week notice with no penalty. $1,500 cancellation fee without 2 weeks notice, for both providers and facility. Facility can cancel 2 shifts per 13 week contract with no penalty - guaranteed hours only apply at 3rd cancellation. Right to hire with no penalty after 13 week assignment completed. Late cancellation of shift by client affiliate or HealthTrust: 3.11.1 Cancelled during a shift or upon arriving to the client affiliate facility for per diem staff – pay subcontractor for the greater of the number of hours worked or 4 hours. 3.11.2 Cancelled before 12 p.m. client affiliate time business day prior to shift for per diem staff - no charge. 3.11.3 Cancelled after 12 p.m. client affiliate time business day prior to shift for per diem staff.
