What You’ll Do

• Participate in all phases of the Case Management Program (assessment, planning, implementation, coordination, monitoring)
• Provide nursing expertise to support case management standards of care
• Identify and integrate local case management processes and opportunities for improved care coordination
• Develop and implement case management strategies across inpatient, outpatient, onsite, and telephonic settings
• Support tools and resources used for patient identification, assessment, guidelines, and community resources
• Coordinate with utilization management and collaborate with social work case management
• Collect and document patient care data accurately and provide patient/caregiver education based on learning needs

What You Need

Required

• Associate Degree in Nursing (from an ACEN- or CCNE-accredited program, or as otherwise identified in the task order)
• Active, unrestricted RN license
• BLS (AHA or ARC)
• Minimum 1 year Case Management experience within the past 2 years
• One of the following certifications: CCM, CDMS, CRRN, COHN/COHN-S, ACCC, CRC, RN-NCM, or CMC
• Ability to pass a government background check and obtain a government security clearance

Preferred

• Experience integrating case management with utilization management and multidisciplinary teams
• Comfort working across onsite and telephonic care coordination workflows
