Care Coordinator

Role: Care Coordinator (LCSW) supporting patient-centered care in an acute care hospital setting. Responsibilities: Conduct psychosocial assessments, develop individualized care plans, and coordinate services across inpatient and outpatient environments. Collaborate with physicians, nurses, and community agencies to facilitate safe discharges, reduce readmissions, and connect patients with resources. Provide counseling, crisis intervention, and education to diverse patients and families. Document in EMR, participate in multidisciplinary rounds, and contribute to quality and safety improvements. Qualifications & Skills: Licensed Clinical Social Worker (LCSW) with expertise in psychosocial assessment, care planning, and patient advocacy. Strong communication and teamwork skills, with a compassionate approach to diverse populations. Ability to work collaboratively in a multidisciplinary environment and adapt to a dynamic healthcare setting. #J-18808-Ljbffr
