Case Manager Registered Nurse

Houston Methodist is seeking a Case Manager Registered Nurse to coordinate patient care across the continuum, ensuring safe, timely transitions and optimal outcomes. In this role, you will perform comprehensive assessments, develop and implement individualized care plans, and collaborate with physicians, therapists, social workers, and payers. You will facilitate discharge planning, navigate insurance and authorizations, and connect patients with community resources. The RN Case Manager supports quality, safety, and cost-effective care while contributing to Houston Methodist’s mission-driven, innovative, and team-based environment.
