Outpatient Physical Therapist

This is a 13-week contract position.

5x8.0 shifts.

The Outpatient Physical Therapist will evaluate all assigned patients, developing, implementing, and modifying treatment plans according to patient needs, goals, and physician direction. Responsibilities include the delegation and direction of therapy services and ensuring the design of functional treatment programs for effective community reintegration. The therapist will serve as an educator to patients, families, students, and coworkers while upholding compliance with organizational standards.

• Conduct comprehensive evaluations of assigned patients.
• Develop individualized treatment plans to progress patients towards discharge.
• Document care accurately and timely while adhering to guidelines.
• Provide supervision and a positive learning environment for students.
• Create and pursue an individualized professional development plan.
• Maintain professional licensure and complete continuing education requirements.
• Participate in performance improvement initiatives.

• Degree in Physical Therapy from an accredited institution.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR Certification.
• Active state licensure for Physical Therapy.
