divh2Hospice Registered Nurse Opportunity/h2pNow Hiring Registered Nurse North Wales, PA/ppHours: PRN Weekends/ppThe Registered Nurse will be responsible for assisting the team with on call/case management/visits to aid in coverage and call outs. Job duties can include, but are not limited to:/polliCompleting an initial assessment of patient and family to determine hospice care needs/liliRegularly re-evaluates the patients nursing needs/liliUse assessment data to determine care plan needs/liliCollaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide care that is continuous and well-coordinated/liliCounsels the patient and family in meeting end of life needs/liliInitiates appropriate preventative and palliative nursing procedures/liliCommunicates with the physician regarding patients needs and reports any changes in the patients condition/liliSupervises the Hospice Aides/li/olpEducation:/ppAssociates Degree in Nursing/ppProfessional Experience:/pulliMust have a State Certified RN license/lili2 years experience as a professional nurse, hospice experience preferred/liliMust have a valid Drivers license and reliable transportation/liliMust be willing to travel/liliStrong verbal and written communication skills./li/ulpAscend Hospice Palliative Care PA - is part of the Ascend family of Healthcare Companies.
